Office Visit Policy

  

Overview

This policy outlines the procedures and guidelines for scheduling, attending, and managing office visits at our primary care practice. It ensures a smooth and efficient experience for all patients.

Scheduling Appointments

Making Appointments

- Appointments can be scheduled by phone, through our patient portal, or in person.

- Patients are encouraged to schedule routine visits at least two weeks in advance.

Same-Day Appointments

- A limited number of same-day appointments are available for urgent needs. Call early in the day to secure a slot.

Check-In Procedures

Arrival Time

- Patients should arrive 15 minutes before their scheduled appointment time to complete the check-in procedure or if they have any changes to their insurance and/or contact info.

Required Documentation

- Bring a valid photo ID, insurance card, and any relevant medical records.

- New patients should complete the registration forms available on the patient portal at least 48 hours before arrival.

During the Visit

Courtesy

 - Patients are expected to treat staff and other patients with respect. We strive to provide a safe and inclusive space for all of visitors and staff.

- We will not tolerate rude or aggressive behavior towards others in our clinic. Doing so will result in immediate dismissal from our practice.

- Mobile phones should be silenced during the visit.

Preparation

- Bring a list of current medications and any questions or concerns to discuss with the provider.

Cancellations and Rescheduling

Cancellation Policy

- Appointments should be cancelled at least 24 hours in advance.

- Repeated cancellations or no-shows may result in a $50 non refundable deposit that must be paid prior to scheduling an appointment and limited future appointment availability.

Rescheduling

- Patients needing to reschedule should contact the office as soon as possible to arrange a new appointment time.

Follow up

After the Visit

- Patients will receive instructions for any required follow-up care or additional appointments.

- Test results and any follow-up communications will be shared through the patient portal or by phone within one week after the visit.

Emergency Care

- For medical emergencies, patients should call 911 or go to the nearest emergency room. Our office can provide follow-up care after the emergency has been addressed.
