Form Completion Policy

  

  

Policy for Completion of FMLA, Short-Term Disability, and ADA Accommodation Paperwork, and Letter of Medical Necessity

At Druid Hills Primary Care, we are happy to assist patients with documentation for FMLA, short-term disability (STD), ADA accommodations, and letters of medical necessity when medically appropriate. To ensure timely and accurate processing, the following policy must be followed:

Patient Requirements – Checklist

✔️ Paperwork Must Be Brought to a Scheduled Appointment

We do not complete forms outside of a visit. You must schedule an in-person or telehealth appointment and bring the required paperwork with you. We cannot print forms from employer or insurance websites. 

✔️ Specialist Records Must Be Provided

If the request is related to a condition primarily managed by a specialist (e.g., psychiatry, rheumatology, neurology), you must bring recent documentation (within the last 6 months) from that specialist.
We will not complete paperwork for conditions not managed by our office unless proper documentation is provided. 

✔️ $50 Forms Completion Fee

 A non-refundable $50 fee applies per form. This fee must be paid prior to completion and is not billable to insurance. 

✔️ $25 Letter Completion Fee

 A non-refundable $25 fee applies per letter. This fee must be paid prior to completion of the letter and is not billable to insurance. 

✔️ Allow Up to 7 Business Days

Forms and letters will be completed within 5–7 business days after your appointment and once all necessary documentation is received. 

✔️ Timeframe Policy for Leave Requests

The length of leave granted is based on the severity of the medical condition and our clinical judgment. Our standard policy is as follows:

  • A maximum of 4 weeks of leave will be granted at one time.
  • Patients must return for re-evaluation every 2–4 weeks to determine if a medical extension is warranted.
  • Extensions will only be provided if medically necessary and supported by ongoing clinical documentation.

Important Notes:

  • We do not complete long-term disability applications, or ongoing certifications for conditions outside our scope of care. These should be directed to the managing provider.
  • If your forms require addendums or modifications, then you will need to schedule another appointment with your provider to make necessary changes.
  • Submitting incomplete forms or missing documentation will delay processing.
  • All communication regarding forms must go through our administrative staff.
